Missed appointments not cancelled or rescheduled with less than 24 hours notice will be subject to a $25.00 office fee. Missed procedure appointments will incur a $50.00 charge. Frequently missed appointments may result in dismissal as a patient.

Returned checks are subject to a fee of $35.00.

Medical records may be obtained by downloading and signing a Medical Records Release form. There is a minimum $10.00 office fee, as per Virginia’s Health Records Privacy statute, which must be paid in full before your records are released. Please allow two weeks for processing.

Disability / Medical forms are available for $25.00. Please allow two weeks for processing.
